
Funding Request Information
- All funding requests are reviewed and voted on annually by the CMS Foundation Board at its June meeting.
- Grant requests and awards are typically $5,000 or less per agency or organization.
- Grant requests are due by May 15; applicants will be notified of their grant request status by June 30.
- Requesting entities should supply the following information:
- A request letter articulating the project description, timeline, and amount requested.
- A list of trustees/directors and key staff.
- Non-profit organizations should attach a copy of their IRS determination letter, documenting tax-exempt status under section 501(c)(3) of the IRS code.
- Upon review, the CMS Foundation Board may request additional information, including most recent financial statements and/or audit.

- Fundraising events
- Requests to cover operating deficits
- Churches, or activities to promote a religious faith
- Memorials
- Political candidates
- Multi-year grant requests
- International projects
- Endowment campaigns
- Grants to individuals
